PROCEDURE FOR REQUESTING RECORDS

 

  1. To request public records from the Department, please complete the fillable form located (here) and send to oidopenrecords@oid.ok.gov.
  2. Your written request should describe the records you seek with as much detail as possible. This description should include the type of record (e.g. financial statements, administrative actions, licensure information, etc.), specific name(s) of the licensee(s) or persons who are the subject of the record, a relevant time frame, and any helpful search terms or other information that would help the Department identify the records you are seeking.
  3. If you need any assistance in filling out your request, please contact the Department at oidopenrecords@oid.ok.gov or call (405) 521-2746. If you email your questions, please include your telephone number and mailing address.
  4. Once the Department receives your request, you will be notified by email that your request is being processed. The Department strives to respond to all record requests as promptly as possible. Please note that the time needed to complete records requests varies depending on the size of the request, the types of records requested, and the current volume of records requests submitted to the Department.
  5. Please note that you may be required to pay a fee before receiving the records. Pursuant to 36 O.S. § 321 and 51 O.S. § 24A.5(3), the copying or reproduction fee is $0.40 per page and $1.00 for each certified page. Additionally, depending on the size and nature of your request, you may be charged an additional fee for time associated with recovering or gathering the records. The recovery fee is $25 per hour. The Department will send you an invoice prior to releasing the records. Payments can be made online at https://www.oid.ok.gov/about-oid/divisions-programs/legal-division/pay-legal-fees-and-fines/  or via check, money order, or cashier’s check. The Department cannot accept cash or credit/debit cards.
  6. As soon as the Department receives payment for the records, the records will be delivered electronically or made available for pickup at the Department’s OKC location, 400 NE 50th Street Oklahoma City, Oklahoma 73105. Certified copies cannot be electronically delivered or faxed. If you would like the copies mailed, you will need to provide a FedEx or UPS shipping number. If you do not have a FedEx or UPS number, please contact the Department and arrangements for mailing the records can be made.
